Check for BIOS updates, if it's an IDE drive make sure that all jumpers are in the correct order. Check cables (power, data) for cracks, damages.

Probably the disk is bad. I had this happen and I inserted another disk and it recognized it.
It is possible that the lens on the disk drive may be dirty. If this is not the case, then your disk drive is probably corrupted.
