It depends- The whole lot is Middle Earth, in which there is Eriador (with Rivendell and the Shire), Rohan, Gondor (where Aragorn, Legolas and Gimli do the fighting) and Mordor (where Frodo destroys the ring, and where Sauron is). Inside these places are cities. The elf one is Rivendell (or Lothlörien, Rivendell is the first one), Helm's deep is where the wall blows up and Minas Tirith is where Legolas kills the elephant.
