The guard outside the cave (Bolor Savel) has the key. Talk to him first and he will become aggressive and attack you. If you attack without talking first he has a tendency to run off and disappear leaving you stuck. If this happens you can quick-travel to another zone (like Bruma) and wait 24 hours to see if he reappears. If not then you pretty much have no choice but to reload an earlier save and try again.
