Break the bottle. The pear was not put in by some magical means and will not come out that way unless you let time and decomp take their toll. The pear grew inside the bottle off of a blossoming bough. The bough fit through the bottle neck which was affixed to the tree.

Dont break the bottle. They have these long skinny forks and knives you can cut up and retract the fruit. Still lots of alcohol left in the fruit.
