If your sim is pregnant, they have a chance of twins although it is not very likely they will. However if you save up enough lifetimee hapiness points you can buy a fertility treatment. It increases your chance of twins and triplets! this does work sometimes as on of my people was a rockstar the got married and got pregnant.
