If you've lost or can't find your policy number just call you insurer. they can usually look find your policy number with alternate identification like drivers license number, name etc.
The policy number should be on the insurance card as well as any bills. If these have been lost, then a call to an agent of the company can get the number for you.

PIN stands for Personal Identification Number. It is a 4 digit number that is specific to every magnetic card (Credit or Debit) As per security policy, a customer is not expected to share his PIN number with anybody. Secrecy of the PIN number ensures unauthorized access to our bank account. If our card is lost and money is withdrawn because of our carelessness and sharing the PIN, the bank would not be liable to pay you back.
To write an application for a lost school ID card, address it to the appropriate school authority, explain that you have lost your ID card, provide your personal details like name, student ID number, and class, request a replacement card, and sign the application. When you first signed up for a BDO debit card, the teller would have given you a piece of paper, and printed in it is a 10-digit number (on top of your name) which is the account number. If you lost the paper, contact their call center (number located at the back of your card) or go to the bank and ask for your account number. Don't forger to take your ID with you for identification.
