During the shower valve rough-in, install a pipe nipple (usually 1/2" male for most supply elbows) at the location desired. Position it so it sticks out past the position of the finish wall by about 1/4". The engagement of of 1/2" pipe thread is about 1/3" so you don't want the nipple sticking out farther than this or the escutcheon won't cover up properly. Wrap the nipple threads with Teflon thread tape.

Complete the shower installation and finish wall.

Check that there is no dust or gunk on the taped threads of the nipple. Turn the elbow onto the nipple by hand.

Final tightening is tricky. Unlike a rough-in connection where you have wrench flats on both threaded pieces, here you have no wrench flats on the elbow and no access to the wrench flats on the nipple. A key consideration is whether or not your main shower flow valve blocks the supply pressure on the elbow when it is closed. If not, the elbow thread seal will have to support the full supply pressure, but if so then the thread seal has to support only a very low pressure.

To tighten the elbow without damaging it, you can use a strap wrench, or a wrench with some rags and padding to protect the finish of the elbow, or you can use a screwdriver in the outlet of the elbow. None of these is an ideal way to get enough torque to seal the full supply pressure if that's necessary.

In the case that the full supply pressure must be supported by the elbow thread seal, then the part I still don't know is whether you need to find a way to support the nipple to avoid torquing the pipe inside the wall too much. Nothing I have found online addresses this concern. Presumably, the elbow supply pipe inside the wall should be firmly supported with some bracing or something during the rough-in part to address this concern but I have no idea.

When finished, check for leaks. Again, this is not as easy as with a rough-in connection, since you are now restricted to a very small hole in the finish wall. Stuff some paper towel in there underneath the pipe nipple, wait for a few minutes or hours, and check for any water drops on the towel.

This seemingly simple option on my shower kit turned out to be the trickiest part to install.
