There is really no way to know unless you have had an ultrasound or radiograph done by a veterinarian.

If you had rads/ultrasound done, then you could count the number of puppies born vs. the number of puppies the vet said the dog would have.

Otherwise, if the dog has been struggling and pushing for over an hour with no puppy produced, get in to the vet's office ASAP!
