On a map: by looking at crinkly blue lines with a name (like 'Mississippi') written next to it. When wandering around in nature: by focusing on locations with a lot of plant growth, which is an indication of the presence of water; or by looking for water birds in the air like ducks.
If you meant to ask 'How do you know in which direction a river flows?' the answer is when looking at a map, it alway flows towards the sea. When standing next to it, throw a few twigs in and see in which direction they disappear.
