To left click on a Mac you need to place the Index finger (Forefinger) on the left side of the mouse and press down firmly. Similarly to right click requires the middle finger to be placed on the right side of the mouse and press down firmly. The choice of fingers used can be varied but the index and middle combination are the most common.
The real question is - How do you left click on the Mac Keyboard without a mouse and with the trackpad left click preferences unchecked? I haven't been able to find out an answer outside of getting a Mac mouse.
Mac OS X does not support the Control Key/left click combination like the Windows operating system does. There are numerous keyboard short cuts (See links below) but as the Ctrl click combination could perform any number of tasks (depending on application in use/ configuration etc.) it is impossible to suggest an alternative without knowing the desired outcome.
Normally a single finger clicking on the trackpad is considered a left click. The trackpad clicking is configured in the Trackpad section of System Preferences. The Secondary Click is what is widely known as a right click.
You must be referring to an old Apple single click mouse. If so, you use keystrokes along with clicks to accomplish this. How exactly I am not familiar with. My advice would be to buy yourself a Magic Mouse and use it. If you are referring to a Magic mouse you left click just like any other mouse.

You need to enable right click. Open System preferences, click on mouse, and make sure the secondary click box is chosen. Choose left or right.
