Visit the makeover mage. In the past, s/he has charged 1k per makeover, but I believe the service is free now. You may want to take 1k with you when you go, though, just in case. The makeover mage is located near the Crafting Guild, which is to the south-west of Falador. Follow the outside southern wall of Falador west until you find it, or simply look for it on the Runescape map. The makeover mage has a flock of sheep right outside his/her door and green frogs hopping around inside of her house. (S/he also changes genders frequently, so you can never tell if s/he's going to be a man or a woman.)

To change your gender, you have to go in and make a new account with the gender you want. There is no way to make an in game sex (gender) change.
