First you need an apple (which is a task in itself!) - there is 2 ways: 1. Go onto a multiplayer server and kill Notch or someone using the name Notch or 2. Use an inventory editor (like TooManyItems) but then again if you had an inventory editor you could just give yourself a golden apple. Providing you want it legit you will have to go with the first option and then go and mine 72 gold ingots which you then turn into 8 gold blocks. Put the apple in the middle of the crafting grid and surround it with golden blocks - Voila! Golden apple. It heals 10 hearts so use wisely. Its annoying that you can only do it legitimately in multiplayer but there are talks that a legitimate method to get them in singleplayer will be implemented in a future update.
1.1 Update you can now make golden apples by surrounding a apple with 8 gold nuggets(which can be made with only 1 gold ingot...gold nuggets can also be gotten by killing pigmen in the nether.)
