You are given them on acceptance of Quest 178, started at the grave at Brigadoon, in the north east of the ruin.

it is in the Castle Trodain dungeon, which can only be unlocked after you get the key from the lady in the snow area.
Alchemise it with 1 piece of Orichalcum and 9 pieces of glass frit, to make Erdrick's Sword. There are other Rusty items in the game, all can be restored into Erdrick's items in the same way. You are taught the recipes by one of the DLC quests, and receive the Rusty Gauntlets from it too.
There are a few, yes, but they are rare. The Rusty items can't be replaced once alchemised, those are about the only items you can't get more of.
