That keypad (stamped with a model # of 31D302 A) goes along with a console single button controller. The whole pakage is called a touch code transmitter model 139.18755. The keypad is programmed so simply that you'll kick yourself.
- Find the console (single button controller) that the keypad is wired to
- Remove the small plastic faceplate above the button using a small flat screwdriver
- You should see a 9v battery inside this compartment (it probably requires replacement while you have it apart)
- You should also see a small black switch on the right side of the opening (it should be labeled "program" or "operate")
- Move the switch to "program"; the red led light will start to flash on the keypad
- Go to the keypad and enter your new 4 digit code (nothing more than 4 digits)
- Go back to the console and move the switch back to "operate"; the red light will stop flashing
- Test your code on the keypad at this point, it should work, if it doesn't, repeat steps 5 thru 8
