It is pretty simple to leave any church, and the Seventh-day Adventist Church is no exception. One of the most commonly-used ways to leave is to simply stop attending church and participating in any way. In most cases, the church where you are a member will eventually drop you from the books, some after contacting you, others after long enough time has past. But if you want to do it the official way, you should contact the secretary of the church where your membership is and request to be removed from the church books. Obviously, someone who is a minor may not have the right to do this without their parents' consent, since some decisions are legally up to their parents, especially in some states or countries. But in general, that's how you do it.
