There is no code ruling on this. I set mine 12" to center. If you are putting in a new outlet in an existing home I would suggest you just match it to the height of the rest of the outlets.
Look for a carpet that compliments the hallway, but unless you can match it perfect, don't try. It is better to have a contrasting color that blends well. Go with a carpet that is of similar height for the best match. When you are cutting the carpet up, leave it long in the doorway so that the installer has enough to work with.
