There are really two distinct answers to this question. The first has to do with how one would seem to read another's mind while the second suggests how it might be possible in actuality.

Mentalists, magicians and fraudulent psychics have long used various methods to simulate mind reading. These methods rely on surreptiously obtaining information in advance to produce the illusion of mind reading. Many of these methods are quite clever and most are amazingly simple. Even if you try to guard against such practices the human mind is easily fooled into believing that mind reading has occurred. This is the central arguments of skeptics against the existence of real mind reading.

However there is a second possible answer. Real mind reading likely does exist, and despite what some skeptics claim, even some magicians and mentalists who are schooled in the deceptive methods admit they have witnessed what appears to be the "real thing". The question then is how might this work?

Scientific research in the past two decades into quantum physics now shows that particles are deeply intertwined and connected although the mechanism is unknown. These deep connections seem to have nothing to do with space and time. So based on real science it does appear that the notion of information transcending space and time is reasonable.

Our extended memories do indeed seem to reach across time as well. There are indications these memories are external to our physical bodies. If it were possible to in a sense share these memories, it would be reasonable to accept telepathy or in other words, mind reading as real. Less evolved species, such as insects, seem to exhibit group mind. It is possible therefore that our species early on developed similar abilities - likely to survive - which have been mostly replaced by our social and technological advancements over time. Those who possess the ability to access this apparently information may be using a very ancient faculty that is quite real although rare in common experience.
