Above the box that the question is in there is a 'report abuse' button. Click on it and select the reason. The report goes to a Community Assistance who evaluates that user and determines if they deserve to be warned or banned.

Wiki answers will not be banned until it does something illegal. To my knowledge, wiki answers is completely legal.
First of all, they're called TRUST points. When someone answers/edits a question, scroll down and hit "recommend user". You can only recommend a user once. You can also recommend yourself... Once!
Usually the merge recommendation goes to Deb. Depending on the amount of requests, someone else may help her.
