Istbat.4.H is identified as a Trojan by AVG as of mid-October, possibly earlier.
Istbat.4.H hides in System Restore, so turn it off before you scan to facilitate removal of the infected files.
I had to run Spybot, download AVG from Grisoft, perform an update to AVG, then reboot into safe mode and run a full system scan.
I also ran Ad Aware on another system before it was clean enough to allow antivirus updates and spyware updates.
After removal, reboot and run AVG again, just to be sure. I ended up manually deleting a couple of files that AVG couldn't.
Run a Windows Update afterwards to patch up vulnerabilities that allow Istbar to operate.

I think that some more info is needed like what operating system you use, have you tried to do it from a bootscan, what virus detection system do you have before this can be answered.
i went to dl AVG anti-virus version 7 and it manages to detect the Trojan and delete most of them... as for those which are unable to heal or delete... try to go to the foler that consists of the infected file and the anti virus shd prompt u abt the Trojan... den.. u can safely delete the Trojan horse... hopes it help...
