Remove the standard knob
This simply pulls straight off (..interject something about straddling and tugging firmly...)
There is a plastic sleeve moulded over the end of the shift lever. The bottom of this is a flange which holds the gear boot up near the knob. There are three vertical ribs on this sleeve which stops the std knob from twisting.
If your replacement knob fits over this sleeve then all you need to do is follow the fitting instructions supplied and you're done.
If you have a MOMO or similar you will need the next step.
Removing the top of the plastic sleeve

About 10mm from the bottom of the sleeve there is a groove around the shifter. Cut around the sleeve in this groove.
Carefully shave down the sleeve from the top removing the plastic from the metal shaft underneath. The shaft is knurled and may damage any blade you use. Don't dig too deep into the plastic as it's very thin and will shave off easily.
I removed two sides and then twisted the sleeve off with a pair of pliars.
