Could be a problem with the starter motor, might be causing it to short out or something
Sounds like one of your door switches is bad.
Most modern cars have an automatic choke. This richen's up the fuel mixture and also increases the rpm slightly. once the car warms up the auto choke switches off and the rpm drop off.
One of your door switches is probably malfunctioning. Find the switches usually on the hinge side attached to the body and use a little wd40 and move it in and out and it may start working. If not the bad one will have to be replaced.

Three way switches do not have a up-on down-off position. With the lights off, reposition the switches so that the handles are all down. This is the start position and depending on how you enter the room and turn on the lights certain switches will be left up when you leave the room. That is just the design of the switches and for the asthetics of the look, all handles down the lights will be out.
Start by turning off power to the switches at the breaker box. Use a multimeter to check for continuity in the wiring of both switches to identify any faulty connections or breaks. Look for signs of wear or damage in the wires leading to the fan and replace or repair as needed.

Off and on switches connect to a current. An on switch allows the current to run through an object, essentially "turning it on". Off switches cut of this current.

I have the exact same problem. I brought my car to Canada from the USA, and it switches over fine, but when the car is off and started up again, it's back to MPH. Grrr....
