In of itself, 'false information' is not generally reportable.
If the false information is libelous or abusive, or is otherwise against Facebook's TOS, you can report it, and you'll report it in different ways depending on the source, but generally there will be a cog icon which has a drop-down menu containing the Report button.

No it does not. The only person who knows who reported them is you and the Facebook employee who is revewing the report.
