Well that depends on what type of jewellery is in the first piercing, it can be three types of jewellery so until one knows what's in there, giving you a clear answer is not going to be easy.

You don't. That's just because most doctors are against piercings. You can always deny a ridiculous request like this and continue surgery with your nipple piercings in.
If you intend on keeping the piercings, jewellery should be left in the piercings, there is no reason to remove the jewellery except to get rid of the piercings. Piercings can take a full year to become seasoned to the point where jewellery can be removed for extended periods.
Well the piercings will not close up completely after that period of time. It can take a year or longer for piercings over a year old to shrink and close.
