Why do you feel the need to do this? If you feel it will upset them to be told this then your instincts are probably correct, it will probably upset them! There is never any point in upsetting people for the sake of it.
If you want to help, if ever you know she is trying to lose weight, encourage her by telling her how well she is doing. Furthermore, I am sure she already knows she is fat. Being fat doesn't mean she has no brain.

Fat people know they are fat,it is not something that can just sneak up on them without being noticed..
If your girlfriend was not fat when you got together with her and she has been gaining weight and you find the fat to be unattractive and a sexual turn off then just tell her the truth straight up.
