Attack mobs while having no weapons equipped. Be warned that your damage output is directly related to your weapon skill (i.e. Unarmed while using no weapon) so I would advise starting with lower level creatures as long as you can.

Open your professions pane with the "K" key, and click on the icon for the profession you wish to level. You will see various recipes, each with a color. By collecting the materials mentioned in the recipe, you will be able to make those items, awarding you increases in skill if it is a recipe that is green or better. Green - sometimes awards +1 skill Yellow - often awards +1 skill Red - always awards +1 skill Red with a (+3) or (+5) - making this item offers 3 or 5 skill points Grey-colored recipes do not offer skill points - you will need to make more difficult items. Once every so often, you will hit the cap of your current profession level. Visit a trainer to increase your skill cap and train new recipes. Just create a new account and do quests that give you defence exp and train just defence skill, then train attack skill.
You don't have to train it. The skill cape is available at any level. And there actually isn't a way to train it.
