With great difficulty. It'lll probably require replacing the hubs, the mounting brackets at the axles, maybe even the head cylinder. There'll be a proportioning valve somewhere that'll need attention too.
It has disc breaks in the rear. I believe the gt one has disc. in front and back and se one has front disc. and rear drum.

There are disc brakes front and back on the Cherokee. EDIT: Cherokees (XJ) have Drum Brakes in the back. Disc in the front i don't know if you were referring to the new Grand Cherokees .. Those have All Disc. Hope it helps. (i own a 88 Cherokee XJ)
Disc brakes do not require adjustment like drum brakes. They are self adjusting by design.
