If you would like to unblock someone from facebook, simply go to your account settings and click on a button that says "Blocking". Once clicked on, you can see a list of people or a person previously blocked and simply click unblock and you are done.
You can't unblock yourself. They have to unblock you by going to your page and hitting "unblock friend" It should be right where it usually says "Block Friend" If they just un-friended you then try re-adding them. If they don't unblock/re-add you they obviously just don't want to be your Facebook friend anymore.
When someone adds you as a friend on Facebook, you have the option of accepting their friend request. When you send out a friend request, you are notified by Facebook if that request has been accepted.

Goto privacy setting. Click Manage Blocking. Then click unblock the people you want to unblock.
You go to your notifications and then look for the friend request. :-)
Type in the name of the person you want to send the request to in the top right corner. Select the right person on the list that will show up and click Add As Friend
You have to get them to send another friend request. You simply just click on your friend tree and type in their name in the friend request box and add them again.
The only way to know if your friend request has been declined is to request to be their friend again. If it lets you re-request, then they denied your friend request.
