You can use one of two ways:
Since Custom Content downloaded from The Sims Exchange is in the proper file format, you can just double-click on the file after it has been downloaded to your computer. The Sims 2 Package Installer will then pop up and allow you the option to quit or automatically install the custom content.

After downloading Custom Content, you must place the file in the Download folder under My Documents\EA Games\The Sims 2. If there isn't a Download folder there, then you can create one. Make sure you spell it right, or it won't work.
After you have done ONE of the above options, you must:
