How do you use the Yu Gi Oh card Polymerization?

First off bit of a stupid question but here goes polymerisation is a card used to fuse two monsters together to create a new monster probably the best example of this would be to use jaden yuki s deck the elemental heroes his deck was full of fusion monsters yugi himself used a few in times of need ie one of my favourite is his dark magician into flare knight. Polymerisation isn't the only card used to fuse monsters together other cards include future fusion, miracle fusion, super polymerisation and others.
