The book is the Oghma Infinium. You need access to a bookshelf.
First you 'use' the bookshelf.
While in the inventory screen, read the book and choose the skill path you wish to increase. Close the book, but do not exit the inventory screen, you need to place the book on the shelf before exiting.
The book is now on the shelf, and did not disappear.
Click on the book and remove it without reading it.
Repeat the process.

You can reach level 81 in Skyrim I think but that will take an awful long time, and to be honest, who would be bothered.

There is a hard cap of Level 81. This is determined by people using a cheat to put all their skills to 100 and seeing what level they become. When all skills are 100, there's no way to level further, so 81 is the cap.

Yes and No. The game is based on a level system. There is said to be an unlimited number of quests. If you get to around level 81-82, you stop leveling up. If by beat, you mean finish the main story line then yes. If by finishing the game beyond playability, then no.
