Legally you can't, as you are directly responsible for the patient's treatment.
If it is a "copy" of the original, it is whatever the date on the ORIGINAL form would indicate.

You can look up examples of disclaimers. They pretty much have a standard format saying you are not responsible for what people might do with the information or product you are giving them. A disclaimer relieves you of legal responsibility if something goes wrong.
You are doing that particular project ,so you should only know how to write it.
We just have to specify what we have learnt from the project or write down the main points....Though it's boring we have to write !
