how to write an application to the bank manager to unblock atm card and new pin issue
You can write a letter to a bank manager for an ATM pin but it is much easier to call the bank's customer service department. They will ask you a variety of security questions and then send out a new card with a new pin. If you decide to write, they will need personal information that you may not want to provide in a letter.
To write an application letter to have your ATM blocked, you have to address it to the bank in question. In your letter you should request that the bank blocks your ATM.

Contact the bank - they'll send you a new PIN to your registered address.
If you can either visit your bank branch or telephone them that would be quicker. If you wish to send a letter, you will need to include your bank account number, the reason why you want a change of PIN and of course your contact details such as your address and telephone number. You cannot block a PIN number but request a new one to be posted to you. You can stop the card should the details of your PIN be in the hands of another.
SBU : request to issue to apply a new card
You can check out the letters you received from the bank to try to find out the PIN number of the ATM card (And that is only if you haven't changed it already). If this doesn't help, call up customer care and identify yourself and then request for a new ATM PIN. They will send you a new PIN within the next few days.
