You write it the same way you write any other story. Sit down and start putting ideas onto paper or into the computer!
Write what it feels like to be addicted, what happens to the person who is addicted, why did they become addicted, and whatever else you can thing of!
Something that inspires you in the story you are reading, or for the author of the story, it's something that inspired him/her to write the story.
Why don't you become addicted to it. Then you write about it when you are addicted to it.

How would you write one without them? The elements are just names for the parts of the story -- they're not something you pick up and plop down to make a story. Write the story, then worry about which elements are which.

Speculation just means thinking about what might happen IF -- you write the story in the same way you write every other story, just write it about "what might happen if" instead of about something that's already happened. Science fiction is the number one speculative story genre.
You know because it's something interesting to you! Anything that interests you is a good idea for a story -- if you try to write about something that you don't care about, your story will be dull and boring. Pick a topic that is so interesting that you want to research and find out more about it -- that's the one to write about.
Then I suggest you write something short like a poem or song.
Think of your biggest fear (like a clown, or something), then write a story behind that fear. Once you get that done, write more stories about it.
First, you write the story. Titles come from the story, not the other way around. Choose something from your writing that gives a description of what happens in the story, or that hints at something important, or that will just attract the reader's attention to it.
