It is not known how each of Bluestar's lives were lost. We do know, however, how she lost her 7th, 8th, and 9th lives.
7th Life - Lost while battling rats in a ditch. (Into the Wild)
8th Life - Lost to greencough. (Fire and Ice)
9th Life - Lost while saving ThunderClan from a dog attack and drowning. (See Related Questions Below) (A Dangerous Path)
It was also stated in "Bluestar's Prophecy" that she had lost three lives due to Thistleclaw, possibly due to murder.

Bluestar loses her first life to a dog attack, her second life to a wound infection after a battle, her third life to a hawk attack, her fourth life in a rockfall, and her fifth and final life after succumbing to an illness.

Seventh Life:
battling rats making her to lose a life.
Eighth Life:
she had greencough.
Nineth and Final Life:
saving her clan by luring the pack of dogs away from Fireheart and Thunderclan.

∙ 13y agoI know she lost her last life while saving her clan from a pack of dogs, and I believe she lost one while fighting rats. I'm not sure about the rest... you can read Bluestar's Prophecy and the first few books in the warriors series for the answer.
