It depends on the car. In carbureted cars it will starve it for fuel, and if you run it for long periods, especially on hot days it can burn up your motor. In fuel injected cars they are hard to start and will sometimes stall. This can also possibly damage your fuel injection system. If at all in doubt just change the filter, it should be changed every 15,000 miles, so your car likely needs one any ways. This is not to say that it is clogged and will destroy your motor, but it couldn't hurt.
