Although the above answer may be correct in that epinephrine increases wakefullness, it is not the reason it's mixed with some anastetics. Further, I don't know why one would want to remain more awake during a dental procedure, and I'm assuming the question was asked becuase some do not like the feeling (or perceive to at least) of epinephrine. The Dr explained to me, the reason why this works is noradrenalin or epinerphrine restricts blood vesels slowing blood flow from the area, which in turn slows down the process of the anestetic getting flushed out of the area and ultimately cleared by the body in whatever way it does so.

Bottom line is a nostril piercing is quick and almost painless, small poke, very fast, your eye might water and then its done.
