The fat (or cholesterol) builds up along the walls of the arteries. If there is too much build up in a particular segment of the artery, it can restrict or inhibit blood flow to the heart. This then causes ischemia and the reduced blood flow can cause damage to the area of the heart which is not receiving enough blood. Cholesterol plaques can also break off and lodge elsewhere in the body causing a similar problem for the heart. They can also travel to the brain and create a stroke.
