Heroin, along with all opiates, affect the eyes by constricting the pupils (the black centers). Somebody high on heroin will have very tiny pupils and the color of their eyes (the iris) will be very pronounced.

Your eyes get watery/teary (roughly 8 hours after use) and drowsy. They turn from vibrant to dull and hollow, pale if a continuing user.

first hand experience will tell you that using ANY opiate for any extended period of time will quite literally take your sole. it starts with memory loss, works its way to your motor functions, strips your body of nutrients. your teeth start to rot. you'll lose a ton of weight. your skin and eyes will start to turn yellow due to inevitable liver damage. you can and probably will get bad acne from constantly touching and scratching your face. the list goes on and on my Friend but the worst thing about opiod use is addiction and withdrawal. i can only tell you that going through heroin withdrawl was the single most horrible experience i have ever had, and i served two terms in the Iraqi/Afghan war.
