I have the same question. The metal cap came off but I can't get the plastic part off. Tried a pair of plyers etc. No luck. just turn bottle upside down and pour. the booze comes out. pretty stupid really.
You use a bottle cap to open the bottlecap. Place the bottle cap on your thumb, make a "C" (or a backwards "C") with your hand, and pop open the cap with an upwards motion.
You will first need to open the fuel flap on the Skoda Superb by pressing on the left side of the flap. Once the flap is open take off the fuel cap by unscrewing it counterclockwise wise.
By placing the cap into boiling or hot water for a moment, you cause the cap to expand more than the bottle it is closing, thus loosening the cap and making it easier to open.

On my Ford Explorer , that is equipped with a locking gas cap , I turn and hold the key while I'm unscrewing the gas cap in order to remove it ( to install the cap I don't need the key , just screw it in place ( and in my case I wait until the cap makes a clicking noise )
press the cap down and than twist counterclockwise to open the bottle lid of theraflu cold n cough syrup.... no. it is literally completely impossible to open this stupid bottle. youll die of a sickness before you manage to open it. you should probably just stab the cap with a knife and suck it out like a champ. otherwise, keep coughing, betch.
I place a piece of damp towel over it ,this gives a better grip.
