For formal occasions utensils should be placed with forks on the left of the plate, knives to the right, and spoons to the right of the knives. Utensils should be placed in order of use from outside to inside, (for example in a meal comprising of a starter salad, then a main course, the salad fork would be on the outside, followed by the dinner fork closest to the plate.) Knives should have the blades facing toward the plate. Dessert utensils are placed horizontally across the top of the plate. Obviously increasingly complex meals will require more complex place settings.

Yes the cornbelt is a formal region.
explain the difference between formal and informal risk assessments
The difference betwen informal and formal theatre is the way you dress and who for. Informal theatre is to be casual while formal is suppose to be fancy. Informal could be in a small classroom while formal would be in a huge auditorium.
You start with soup first. You don't need to worry as someone will serve you dinner in the right order. When in doubt, look at the order in which the utensils are displayed, from the outside going toward the plate. That will normally give you the necessary clue.
Yes, at a formal dinner appetizers are required to be served.
A formal dinner
The best place to purchase a formal dinner jacket will be through retailers who sell fancy and formal dress wear. Retailers such as David's Bridal, The Men's Warehouse, and Karen's Bridal are just some of the many outlets a formal dinner jacket can be purchased.

The dinner knife, or 'service knife,' in a semi-formal, or formal setting would be for the knife to be at the right of the dinner plate, with the blade facing the plate. If there is a butter plate and butter knife or 'spreader,' that knife should be on the butter plate to the left of the dinner plate, just above the cutlery on the left. The butter knife should be place with the handle facing to the right and the blade facing downwards. If there is an additional knife, such as a fish knife, etc., that knife should be to the right of the dinner knife, with the blade facing towards the dinner knife.
In a formal dinner setting utensils are either on the left or right of the plate. The knives and spoons are set to the right of the plate and the forks are set to the left of the plate. If there is more than one fork like a salad fork, the salad fork will be placed further left to be used first. The same applies to a soup spoon which will be placed further right of the plate to be used first.
Macy's and Davids Bridal are both good places to find formal wear and evening attire. J.C. Penney also has formal wear that would be appropriate for a dinner party.
