she realizes she can trust him, and he was trying to help her, but i really think the reason Peeta went with the careers was because he didn't want to die, he knew no-one would attack him if he was with them so he joined them.
She uses it as a tourniquet for Peeta's bad leg. She then frees it from the torniquet, with Peeta's permission, and uses it in sympathy to put Cato out of his misery and end his life.
By playing up to the crowds Katniss gets more sponsors which helps her get life saving supplies in the Arena.

When Peeta saves Katniss, it affects her because she feels like she owes him more than he owes her (if that makes sense); Katniss feels that he has done more for her than she can ever make up to him.
