Volcanism forms when two oceanic plates collide, causing the more dense one to subduct under the other. When it goes under, it partially melts and the magma forces upwards to create an active volcano. An active volcano will stay active until cut off from the magma. Also, the more the volcano erupts, the more crust will form, so if a volcano was once under water, an island will eventually form if not cut off from magma.
