The Appalachian Mountains formed about 480 million years ago. They were formed in tectonic plate collisions that eventually led to the birth of the Pangaea continent, and used to extend to what is now Morocco.

The Americas originally started out in S. America about 23 million years ago. like most other ground sloths when the Americas first connected they moved up north. ground sloths were some of the only fauna that moved to N. America, many of the N.America species moved south though. the giant ground sloth died out or became extinct 10,000 years ago.
The Andes Mountains are locaed in South America. The Andes formed during the Cretaceous period (about 138 million to about 65 million years ago) when the Pacific crustal plate began to slowly slide beneath the South American plate, uplifting and folding the sedimentary rocks that comprise the Andes (see Plate Tectonics).

Basically. Back in Pangea the continents were connected and those two mountain ranges which were originally one. But over the millions of years magma rised between the South American Plate and the African plate (basically right in the middle of the atlantic ocean between Africa and South America) and cooled, causing them to move away from each other, splitting the mountain range in two.
Clearly the Western Hemisphere called the "New World" had been occupied by Native American Tribes and civilizations for thousands of years before Europeans began to make settlements in the Western Hemisphere. Scientists believe that thousands of years ago peoples from Asia had crossed into North America and moved South all the way into southern South America.
