In the US and Canada, until 1995, all area codes had 0 or 1 as the middle digit. In most parts of the country, a long-distance call within the same area code could be dialed as, for example, 1-555-0123 instead of 1-402-555-0123. The only overlay was area code 917 on New York City's area codes 212 and 718. (310 was originally planned as an overlay on 213, but it was changed to a split.)
Now, area codes can have any middle digit except 9 (reserved for future expansion). If you dial "one plus," you must always dial the area code. Overlays are becoming more common, especially in Canada: by the middle of 2013, all of B.C., Alberta, Saskatchewan, Manitoba, and Quebec will have overlays, along with all but one of Ontario's area codes. In overlays, you must always dial the area code on all calls, even local calls to a number with the same area code.
