After the Amidah prayer, as the congregation says the "Brikh Shemei" prayer, one of the congregants opens the Holy Ark. He takes out the Torah scroll and slowly carries it to the Bimah, as the congregants kiss it along the way. The scroll is set down on the Bimah (a special table), and the cloth cover of the scroll is pulled off (see the attached Related Link). Then one of the congregants is called up to the Torah-reading, and after he says the blessing, the scroll is opened.
It varies, depending on which service is being held. In a Friday night service, the holy ark is opened once, but the Torah is not removed or read.

I'm not sure what you mean by "page turner". The torah is in scroll form, and it's opened to the entire portion when its read. So no pages are turned. The reader is called a ba'al kri'ah (בעל קריאה), or ba'alat kri'ah (בעלת קריאה) for women.
