Pollution may occur anywhere there is human activity : agricultural, industrial, or recreational. We do not normally include as "pollution" the similar damage done by natural causes (mineral leeching, weather, overpopulation of plants and animals). In the US, the Environmental Protection Agency (EPA) regularly monitors water quality in many areas of the country. This is especially important in areas where pollution has been found and/or addressed. The discharge of sewage, chemicals, and heated water is restricted by law, but this is not always sufficient to avoid damage to the ecosystem.
