I'm not sure what the real answer to this question is, but from personal experience I can tell you that I went 5 days and was perfectly focused. Towards the end of the 5th day my brain started crashing, weirdest experience of my life. My brain would turn off however my body would not. I would "fall asleep" randomly however the rest of my body was still in the same position when i "awoke," my head did not even bow down, it was in the same position. I also did not take good care of my body during those 5 days, I was only averaging one meal a day and almost no water intake. I'm sure if you took good care of your body then you would be able to pull another day or two more.(6-7 days)
The effect has to do with the inherent stimulant properties in the drug. When Adderall or other amphetamine-based drugs hits the pleasure center in your brain, it releases a level of dopamine many many times higher than normal, which raises blood temperature, pressure, and enables the body and mind to perform well beyond its normal capacity. In higher doses, it prolongs the body's endurance and pushes it beyond its normal threshold. Speaking as someone who did the illegal variety when he was younger, and is now on Adderall because of the damage the other version wreaked on his brain (in the form of severe adult onset ADHD), the same is also true of methamphetamine, benzedrine and the other contraband stimulants, but to a more extreme scale. Health-wise, it's not good for your body to be awake more than 36 hours without some form of rest or rejuvenation, and then, only if your body is in supreme working order. My record for staying up on the other was 21 days (not a lie; a fact) and on Adderall, around 8 days; both are awful for your health and can cause mid- and long-term problems, such as insomnia, high blood pressure, and in the long stretches, depression, fantasy-reality inversions, and psychosis.
