Usually a horse has only one foal. Being a very large animal it is rare for a horse to have more than one at a time. However, twins are not unheard of. Twins do not usually live, one of them is usually weaker.
Typically, a mare usually gives birth to one foal at a time. Occasionally, twins are born, but this is quite dangerous for the mare and very rare. I have never heard of triplets being born, as I doubt there would be enough room in the womb for three foals.
This depends: If you are asking about a single mare with one pregnancy she will most-likely have one foal. Twinning can occur in horses, but is uncommon.
