he runs into the burning church to save the kids in the fire. then as they are running out with the kids a wall falls on johnny when he is escaping from the burning church. so then his back is broken and if he did live then he would not ever be able to walk again. so johnny dies because of the fire.

I am guessing this is after he came home. He was still not fully recovered from the smoke inhalation he got from saving those kids from the burning church. The other case is that he was suffering from not only the smoke inhalation but along with a concussion he got from the rumble, and adding to the fact he watched both Johnny and Dally die right before his eyes.
