Officially only one person died at Woodstock (the original one in 1969). Physical damage (hurt) is a hard thing to measure.
I believe there were three people physically hurt bad enough to require medical attention, however, any time you get that many people together many small injuries happen that do not get officially noticed.
The was also a tent run by the Hog Farm (a communal group lead by Wavy Gravy - they also ran the food "kitchen") that was designated and used to help people who were experiencing "bad trips," but there is no official record of how many people they "treated."
